Drug Shortage: Non-preserved, Bisulfate-Free Epinephrine - December 2012

Non-preserved, bisulfate-free epinephrine is the basic component of the intracameral epi-Shugarcaine solution used for intraoperative floppy iris syndrome (IFIS) and pupil dilation. Earlier in 2012, American Regent, which was the sole source in United States for non-preserved, bisulfate-free epinephrine, suspended its manufacture of drug products. As of December 18 2012, this product is still not available from American Regent, although manufacturing has resumed and product is expected to be restocked but they have not issued a resupply date. This clinical statement provides options for ophthalmologists to employ during this shortage, as well as links to check drug availability and resources on compounding pharmacies.

Instrument-Based Pediatric Vision Screening Policy Statement - October 2012

A policy statement describing the use of automated vision screening technology (instrument-based vision screening) is presented. Screening for amblyogenic refractive error with instrument-based screening is not dependent on behavioral responses of children, as when visual acuity is measured. Instrument-based screening is quick, requires minimal cooperation of the child, and is especially useful in the preverbal, preliterate, or developmentally delayed child. Children younger than 4 years can benefit from instrument-based screening, and visual acuity testing can be used reliably in older children. Adoption of this new technology is highly dependent on third-party payment policies, which could present a significant barrier to adoption. Pediatrics 2012;130:983–986
